Test Overview

What Is the Anti TPO Test?

The Anti-TPO test is a blood test. It checks whether your immune system is making antibodies against your own thyroid gland.

Your thyroid gland makes hormones that control your body's energy and metabolism. To make these hormones, it uses an enzyme called thyroid peroxidase. In some people, the immune system mistakenly attacks this enzyme. The Anti-TPO test detects those antibodies in the blood.

If the result comes back positive, it usually means you have an autoimmune thyroid condition — most commonly Hashimoto's thyroiditis or Graves' disease. Your doctor uses this result to understand why your thyroid may not be working properly.

The test itself is simple. A small blood sample is taken from your arm. It takes only a few minutes and has no significant risks.

Why Is the Anti TPO Test Done?

Your doctor may order the Anti-TPO test if your thyroid function tests - TSH, T3, or T4- come back abnormal. The test helps find out whether an autoimmune reaction is causing the problem.

It is also used in these situations:

  • To diagnose Hashimoto's thyroiditis or Graves' disease
  • To keep track of a known autoimmune thyroid condition
  • To investigate unexplained infertility in women
  • To check pregnant women for thyroid problems that can develop after delivery
  • To assess thyroid health in people with a family history of thyroid disease

Sometimes it is ordered together with other autoimmune blood tests. For example, if joint pain or swelling is also present, they may recommend an Anti CCP test to check for rheumatoid arthritis. Similarly, if lupus or connective tissue disease is suspected, an Anti DS DNA test may be ordered alongside it. Getting these tests together gives a complete picture of your immune system's activity.

What Is Anti TPO? - Meaning Explained Simply

Anti TPO meaning: TPO stands for Thyroid Peroxidase, an enzyme produced by the thyroid gland. "Anti" means antibodies working against it. So, anti TPO antibodies are proteins your immune system creates that mistakenly attack your own thyroid enzyme. A healthy person has little to no anti TPO antibodies in their blood. When these levels rise, it signals that the immune system is confused and damaging the thyroid gland over time.

What Does It Mean If Anti TPO Is High?

High anti-TPO means your immune system is attacking your own thyroid gland.

You may not feel anything wrong at first. Many people with high anti-TPO live normally for years before any symptoms show up. But the damage keeps building quietly. Over time, the thyroid weakens and may stop producing enough hormones. This condition is called hypothyroidism.

The three most common conditions linked to high anti-TPO are:

  • Hashimoto's Thyroiditis - the thyroid slowly gets damaged and becomes underactive
  • Graves' Disease - the thyroid gets overstimulated and becomes overactive
  • Postpartum Thyroiditis - the thyroid gets inflamed after childbirth

If your anti-TPO is high, see a doctor soon.

Is TPO the Same as TSH?

No - TPO and TSH are completely different. Here is a simple comparison:

                                                        Anti TPO                                                                          TSH

TSH tells your doctor how well the thyroid is working. The anti TPO antibody test tells your doctor why the thyroid may be failing. Both tests are often ordered together for a complete thyroid evaluation.

How Much Anti TPO Is Normal? (Anti TPO Normal Range)

The generally accepted anti TPO normal range is:

Anti TPO Level (IU/mL)                 Interpretation                                  What It May Suggest
Less than 35 IU/mLNormalNo autoimmune thyroid activity
35 – 100 IU/mLMildly ElevatedBorderline autoimmune activity  monitor closely
Above 100 IU/mLHighStrong indication of Hashimoto's or Graves' disease

Note: Reference ranges may slightly vary between laboratories. At JDAR Path Lab, your anti TPO test report clearly displays the reference range alongside your personal result so there is no confusion.

What to Do If Anti TPO Is High? (High Anti TPO Treatment)

If your anti-TPO levels are high, here is what to do next:

  1. See an endocrinologist - Do not take any medicine on your own. A specialist will look at your full report and tell you exactly what is needed.
  2. Get a few more tests done - Your doctor will likely ask for TSH, Free T3, Free T4, and a thyroid ultrasound. These tests show whether the gland is already affected and by how much.
  3. Take medicine if your doctor prescribes it - If hypothyroidism is confirmed, you will most likely be given levothyroxine. It is a small daily tablet that replaces the hormone your thyroid is no longer making enough of.
  4. Go for regular check-ups - Even if you feel fine right now, high anti-TPO means the risk is there. A simple blood test every 6 to 12 months is enough to stay on top of it.
  5. Be mindful of your diet - Some studies suggest that reducing gluten and eating anti-inflammatory foods may help lower antibody levels over time. It is not a treatment on its own, but it can support your overall thyroid health. Talk to your doctor before making any big changes to your diet.

How to Reduce Anti TPO Antibodies?

There is no cure that completely eliminates anti-TPO antibodies. But certain steps, taken under your doctor's guidance, can help slow down the damage and bring the levels down over time:

  • Take your thyroid medication regularly - Skipping doses or stopping on your own can make things worse.
  • Check your Vitamin D levels - Low Vitamin D is linked to higher antibody levels. If you are deficient, your doctor may suggest supplements.
  • Ask your doctor about Selenium - It is found naturally in Brazil nuts, eggs, and fish. Some studies suggest it may help reduce antibody levels.
  • Consider a gluten-free diet if advised - This is especially relevant if you also have celiac disease. Do not cut out gluten on your own without medical advice.
  • Keep stress in check - Long-term stress can make autoimmune conditions worse. Simple habits like regular sleep, light exercise, and short breaks during the day can help.
  • Stop smoking - Smoking is a known trigger for autoimmune thyroid disease. Quitting is one of the most direct things you can do for your thyroid health.
  • Do not skip your follow-up appointments - Regular blood tests are the only way to know if things are stable or getting worse.

An Anti CCP test can help rule out rheumatoid arthritis, while an Anti DS DNA test can screen for lupus - both common companions to thyroid autoimmunity.

Benefits of the Anti TPO Test

Getting the anti-TPO test done can be genuinely useful for several reasons:

  • Catch the problem early - The test can detect autoimmune thyroid disease before any symptoms show up, giving you and your doctor a head start.
  • Find the real cause - Abnormal thyroid results alone do not tell the full story. The anti-TPO test helps your doctor understand why the thyroid is not working properly.
  • Get the right treatment - Knowing whether the cause is autoimmune helps your doctor choose the right medicine and the right dose from the start.
  • Stay safe during pregnancy - Thyroid problems during and after pregnancy can affect both mother and baby. This test helps your doctor keep a close watch when it matters most.
  • Know where you stand -If your levels are normal, that is reassuring. If they are not, you can start taking steps early. Either way, you are not in the dark.

Symptoms or Conditions That May Require This Test

Your doctor may suggest an anti-TPO test if you have any of these symptoms:

  • Unexplained weight gain or sudden weight loss
  • Feeling tired and weak all the time
  • Hair thinning or hair falling out
  • Irregular, heavy, or missed periods
  • Always feeling too cold or too warm without reason
  • Visible swelling in the neck
  • Feeling low, anxious, or having unexplained mood swings
  • Dry skin or brittle nails
  • A fast or irregular heartbeat
  • Trouble getting pregnant
  • A family history of thyroid or autoimmune disease
  • Abnormal TSH, T3, or T4 results in a recent blood test

Test Preparation: How to Prepare for the Anti TPO Test

The anti-TPO test needs very little preparation. Just keep these things in mind:

  1. You do not need to fast - You can eat and drink normally before the test. That said, follow your doctor's instructions if they advise otherwise.
  2. Tell your doctor what medicines you are taking - Biotin supplements (Vitamin B7) in particular can affect the results. Mention any other supplements or medicines too.
  3. Wear a loose or half-sleeved top - It makes it easier for the technician to draw blood from your arm.
  4. Drink enough water before you go - Being well-hydrated makes the veins easier to find and the blood draw quicker.
  5. Carry your doctor's prescription or referral slip - Most labs will ask for it before running the test.

Sample Required

A small amount of blood is drawn from a vein in your arm. It takes less than 5 minutes and causes little to no discomfort.

JDAR Path Lab also offers home sample collection. A trained professional comes to your home at a time that works for you,no need to visit the lab.

Understanding Your Anti TPO Test Report

Your report will show your anti-TPO antibody level in IU/mL along with the normal reference range. If the level is above the normal range, it does not automatically mean something is seriously wrong. Your doctor will look at the number alongside your symptoms and other thyroid test results before drawing any conclusion.

Do not try to interpret the report on your own.

If your doctor thinks another autoimmune condition may also be present, they may ask for a few additional tests.

such as:

  • Anti CCP test - for rheumatoid arthritis
  • Anti DS DNA test - for lupus and systemic autoimmune disease

Our Story - A Lab Built on One Doctor's Passion

JDAR did not start as a big corporate chain. It started with one person who genuinely cared. Her dedication is the reason JDAR holds NABL accreditation and meets international quality standards today.

In 2005, her son Dr. Saurabh Goel, M.D. (Microbiology), joined the laboratory. He brought with him fresh expertise in Microbiology and Immuno-Assays, along with a focus on faster report delivery and better patient communication. Under his leadership, JDAR embraced modern diagnostic technology while never losing the personal touch that made it special.
