What is the Bilirubin Indirect Test?

 

The bilirubin indirect test checks how much unprocessed bilirubin is in your blood.

Here is what happens inside your body. When old red blood cells break down, they release a yellow substance called bilirubin. At this stage, bilirubin is called "indirect" because the liver has not processed it yet. Once the liver picks it up, it converts it into "direct" bilirubin, which the body then removes through bile.

This test measures only the indirect part - the bilirubin still waiting for the liver to process it.

If this number comes out high, it usually means one of two things. Either the liver is slow in processing bilirubin, or red blood cells are breaking down faster than normal.

When you get a bilirubin blood test, your report will show three values - total, direct, and indirect. The indirect value is often the most important one for finding the root cause of a problem.

 

Why is the Bilirubin Indirect Test Done?

 

Doctors order this test when they want to know why bilirubin levels are high - especially when a patient has jaundice.

Jaundice has many causes. A high indirect bilirubin level tells the doctor that the problem is either with red blood cells or with how the liver processes bilirubin in the early stage.

Your doctor may order this test if:

  • You have symptoms of haemolytic anaemia - a condition where red blood cells break down too fast
  • You are a newborn with jaundice - to find out the exact cause
  • You have sickle cell disease or thalassaemia - to track how the condition is progressing
  • You have Gilbert's syndrome - a harmless condition where indirect bilirubin stays slightly high
  • Your doctor wants to check your liver health as part of a full Liver Function Test (LFT)
  • Your CBC report showed something unusual with red blood cells and needs further investigation

  • Identifies the type of jaundice - haemolytic jaundice (blood-related) and hepatic jaundice (liver-related) need different treatment. This test tells which one it is.
  • Detects Gilbert's syndrome - a common, harmless genetic condition. If you have it, your indirect bilirubin will stay slightly high, but there is nothing to worry about.
  • Points to the right treatment - high indirect bilirubin and high direct bilirubin have different causes. Knowing which one is high helps the doctor choose the correct treatment.
  • Monitors blood disorders over time - if you have sickle cell anemia or a similar condition, this test is used regularly to track changes.

 

Symptoms or Conditions That May Require This Test

Your doctor may suggest this test if you have any of the following:

  • Skin or eyes look yellowish (jaundice)
  • Feeling very tired, weak, or short of breath for no clear reason
  • Urine is darker than usual, or stools look pale
  • A blood test (CBC) showed anemia, but the cause is not clear
  • Someone in your family has a blood disorder
  • You are a newborn with jaundice in the first week of life
  • A routine health check showed slightly high bilirubin, but you have no other symptoms

 

Test Preparation

This test needs very little preparation.

In most cases, you do not need to fast. But if your doctor has also ordered a Liver Function Test (LFT) or kidney function test along with this one, fasting for 8 to 10 hours may be needed.

A few simple things to keep in mind:

  • Do not eat a heavy meal the night before
  • Drink enough water and stay hydrated
  • Tell the lab staff about any medicines, supplements, or herbal remedies you are taking  some of these can affect your bilirubin results

 

Sample Required

A small amount of blood is taken from a vein in your arm. The whole process takes just a couple of minutes.

The blood sample is then sent to the lab, where the indirect bilirubin level is measured separately from the total bilirubin.

For newborns, a small prick on the heel is used instead of a vein draw.

Here is a simple bilirubin indirect normal range reference for adults:

What is a normal indirect bilirubin level?

In adults, a normal indirect bilirubin level is between 0.2 and 0.8 mg/dL.

If your result is in this range, it means red blood cells are breaking down at a normal rate and the liver is processing bilirubin properly.

Keep in mind - reference ranges can vary slightly between labs. Always check the normal range printed on your own report before comparing.

 

 

Bilirubin indirect 0.15 and 0.16 means – is it low?

A result of 0.15 or 0.16 mg/dL is slightly low, but this is usually nothing to worry about.

Low indirect bilirubin simply means red blood cells are breaking down very slowly. On its own, this is not a problem.

It only needs further investigation if other blood test results - like your CBC - are also showing something unusual.
