A bilirubin test is a basic blood test. It tells how much bilirubin is in your blood. Too much of it usually means your liver is not clearing it properly.

Bilirubin is a yellow substance your body makes when old red blood cells break down. Your liver picks it up, processes it, and sends it out of the body through bile and stool.
When the liver cannot keep up, bilirubin starts building up in the blood. That is when your skin and eyes start turning yellow. This condition is jaundice.
This test measures bilirubin in three ways:
Direct bilirubin - already processed by the liver and ready to leave the body.
Indirect bilirubin - not yet processed, still moving through the bloodstream.
Total bilirubin - the combined number of both types above.
A bilirubin test checks how much bilirubin is in your blood.
Doctors order it in two situations. Sometimes it comes as part of a full Liver Function Test (LFT). Other times it is ordered on its own when a patient has signs of jaundice, liver trouble, or a blocked bile duct.
The result, called serum bilirubin, tells the doctor whether your liver is processing waste normally or struggling to keep up.
Doctors order this test for different reasons depending on the patient.
Newborns are checked for jaundice shortly after birth. Adults get tested when there are signs of liver disease, hepatitis, or cirrhosis. If a bile duct blockage is suspected, this test helps confirm it. Patients already on liver treatment are monitored with it regularly.
It is also ordered when other liver markers are already off. If your SGPT (ALT) or SGOT (AST) results are high, a bilirubin reading helps the doctor understand the full picture.
Same goes when Alkaline Phosphatase (ALP) is elevated. Adding bilirubin to that gives a clearer view of how the liver is functioning.
If you are getting Hepatitis B & C Screening done, bilirubin is often checked at the same time. Both infections put stress on the liver, so measuring bilirubin alongside them makes sense.
What is a normal bilirubin level?

High bilirubin means one of three things. Either the liver is not processing it fast enough, too many red blood cells are breaking down at once, or something is blocking the bile duct. The most visible sign is yellow skin or eyes.
Low bilirubin is usually nothing to worry about. It simply means red blood cells are breaking down at a normal rate and the liver is clearing bilirubin without any trouble. In rare cases, very low levels can be linked to certain medicines, but this is uncommon.
When bilirubin stays high for too long, the body starts showing clear signs. Skin and eyes turn yellow. Urine becomes dark. Skin feels itchy. Fatigue sets in. In severe cases, especially in newborns, high bilirubin can affect the brain.
In adults, a level above 3 mg/dL is already a signal that something needs attention. Levels above 10 to 15 mg/dL are considered dangerous and need immediate medical care.

Bilirubin starts in the spleen. When old red blood cells break down there, bilirubin is released into the bloodstream.
It then travels to the liver. The liver processes it and sends it into the digestive system to leave the body.
So the spleen produces it, but the liver does the main work. When the liver cannot clear bilirubin properly, it builds up in the blood. Most of the time, elevated bilirubin points to a liver problem or a blocked bile duct.

For jaundice, it directly measures the bilirubin causing the yellow color in skin and eyes. That makes it one of the first tests ordered.
For fatty liver, bilirubin alone is not enough. Fatty liver is usually confirmed through an ultrasound along with blood tests like SGPT (ALT) and SGOT (AST). Your doctor may also add Gamma GT (GGT) and Total Protein & Albumin to get a more complete picture of how your liver is doing.

One important thing: do not read the numbers on their own. Always share the full report with your doctor. The values only make sense when looked at together with your symptoms and medical history.
